Why we built it

Families caring for an elderly relative juggle years of lab PDFs, reports and medication lists. At a doctor's visit it is hard to recall which value changed and when. This tool gathers scattered documents onto one timeline and ties every fact to its source.

Features

Daily logging

A quick form for blood pressure, pulse, temperature, SpO2, glucose, symptoms and falls. Values not measured are never recorded as zero.

Medication tracking

Active medication list, schedule and a log of taken and missed doses.

Document import

Lab PDFs and Markdown records are extracted as drafts and join the trends only after review.

Trends and timeline

Lab values, medication changes and emergency visits on one chart. It shows co-occurrence only and never claims causation.

Visit summary

Medications, measurements, recent labs and open questions for a chosen period, on one page.

Data quality

Flags missing, conflicting or duplicate records; corrections keep their history.

From document to summary

  1. Upload

A PDF or Markdown file is added and checked for duplicates by hash.

  2. Verify

Extracted values are shown next to the source and approved.

  3. Merge

Approved records join daily measurements on one timeline.

  4. Summarise

A sourced visit summary is produced; nothing is final until the user approves it.

Privacy and safety

  • Runs only on the local computer and is never exposed to a network
  • Health data is never sent to external AI APIs
  • It does not diagnose or suggest treatment or doses, and never replaces a physician
